U VISA Application
Instructions:
Complete the entire application, leaving no blanks. Including any and all information you think would be helpful in making a decision on the application itself in blanks where that information is needed. Follow the instructions as the packet states throughout the application.
Attach all relevant case documents – you will have to contact the arresting agency and complete an open records request for this information. We will not fill out any blanks for you.
Once your app is completed you may send it via mail, email, or drop it off at our office.

If your application has blanks it will not be reviewed for a decision.
Once a decision has been made, you will receive correspondence in the form of which you submitted your original application. Turnaround time on applications will vary depending on information we receive from you.
Instructions for Completing and Submitting the I-918B Supplement
Please ensure that you fully complete the I-918B Supplement before submitting it to our office. This includes:
Answering all questions, including those requiring additional explanation.
Providing the case or cause number.
Including your contact information and mailing address.
Using the most recent version of Form I-918B to ensure it is not expired.
